This is a combination regimen of **veliparib** (a small-molecule PARP inhibitor), **carboplatin** (a platinum-based cytotoxic chemotherapy agent), and **gemcitabine** (a nucleoside analog antimetabolite chemotherapy drug). Veliparib inhibits poly(ADP-ribose) polymerase (PARP), leading to impaired DNA repair and selectively enhancing cytotoxicity in tumors with defects in DNA repair mechanisms, especially those with BRCA mutations. Carboplatin causes DNA crosslinking and apoptosis through DNA adduct formation. Gemcitabine incorporates into DNA and inhibits ribonucleotide reductase, disrupting DNA synthesis and repair. The combination has been investigated primarily for the treatment of refractory germ cell tumors and advanced ovarian cancer, especially in patients with BRCA mutations. The regimen has demonstrated tolerable safety in combination, but evidence for efficacy is limited in aggressive refractory testicular GCTs; there is more promising activity in platinum-sensitive high-grade serous ovarian cancer, particularly with germline BRCA mutations[1][2][3][4].
