Venetoclax + azacitidine + donor lymphocyte infusion is a combination therapy used primarily for the treatment of relapsed or refractory acute myeloid leukemia (AML) following allogeneic hematopoietic stem cell transplantation (allo-HSCT). Venetoclax is a small molecule inhibitor of B-cell lymphoma 2 (BCL2), promoting apoptosis in cancer cells. Azacitidine is a hypomethylating agent that inhibits DNA methyltransferase, leading to DNA hypomethylation and cytotoxicity in abnormal hematopoietic cells. Donor lymphocyte infusion (DLI) involves the administration of white blood cells from the original stem cell donor to enhance graft-versus-leukemia effects by boosting immune-mediated destruction of residual leukemia cells. This combination has shown efficacy and tolerability in patients with relapsed AML after allo-HSCT, with reported objective remission rates above 60% and manageable adverse events[1][2][3].
