**Venetoclax + carfilzomib + dexamethasone** is an investigational combination regimen used primarily in clinical research for **relapsed or refractory multiple myeloma (RRMM)**, especially in patients with the t(11;14) translocation. Venetoclax is an oral, highly selective BCL-2 inhibitor that promotes apoptosis of myeloma cells. Carfilzomib is an intravenous second-generation proteasome inhibitor that disrupts protein homeostasis and enhances apoptosis, partly by upregulating pro-apoptotic proteins like NOXA, which increases myeloma cells’ dependency on BCL-2. Dexamethasone is a synthetic glucocorticoid that modulates immunity and increases BCL-2 priming by upregulating BIM, enhancing the effectiveness of BCL-2 inhibition. By combining these drugs, the regimen targets complementary pathways to increase myeloma cell death. The combination has shown high overall response rates in clinical trials, particularly among patients with the t(11;14) chromosomal translocation[1][2][3][6].
