Overview

This sequential combination therapy regimen is being evaluated for the treatment of elderly patients with untreated acute myeloid leukemia (AML). The regimen consists of an induction phase using the VCA combination (Venetoclax, Chidamide, and Azacitidine) followed by a consolidation or subsequent phase using the D-MAG regimen (Decitabine plus Mitoxantrone, Cytarabine, and G-CSF). Venetoclax targets BCL-2 to induce apoptosis, Chidamide is an HDAC inhibitor that modulates gene expression, and Azacitidine and Decitabine are hypomethylating agents that reverse epigenetic silencing. The MAG component includes Mitoxantrone (a topoisomerase II inhibitor), Cytarabine (an antimetabolite), and G-CSF (used to prime leukemic blasts or support myeloid recovery). This multi-targeted approach aims to overcome resistance and improve response rates in a high-risk elderly population.
