Venetoclax + decitabine is a combination therapy used primarily for the treatment of acute myeloid leukemia (AML), especially in elderly patients or those ineligible for intensive chemotherapy. Venetoclax is a potent, selective oral inhibitor of B-cell lymphoma 2 (BCL-2), an anti-apoptotic protein that helps cancer cells evade programmed cell death. By inhibiting BCL-2, venetoclax promotes apoptosis in malignant cells. Decitabine is a hypomethylating agent that incorporates into DNA and inhibits DNA methyltransferase, leading to hypomethylation of DNA and reactivation of silenced genes involved in cell differentiation and apoptosis. The combination has demonstrated improved molecular response rates, higher remission rates, and increased overall survival compared to monotherapy with either agent alone[1][3][5]. This regimen is approved by the FDA for newly diagnosed AML in adults aged 75 years or older or those with comorbidities precluding intensive induction chemotherapy[5]. It has also shown efficacy in myelodysplastic syndromes (MDS)[3].
