Overview

This is an investigational combination conditioning regimen consisting of venetoclax, fludarabine, and melphalan (VFM). It is being evaluated by the First Affiliated Hospital of Zhejiang University as a reduced-intensity conditioning (RIC) regimen for patients aged 50 and older with acute myeloid leukemia (AML) or myelodysplastic syndrome (MDS) undergoing allogeneic hematopoietic stem cell transplantation (Allo-HCT). Venetoclax is a selective BCL-2 inhibitor that induces apoptosis in leukemic cells. Fludarabine is a purine analog that provides immunosuppression to facilitate donor cell engraftment, and melphalan is a potent bifunctional alkylating agent that provides myeloablation and anti-leukemic activity. The combination aims to reduce post-transplant relapse rates compared to standard fludarabine and melphalan (FM) regimens while maintaining a manageable safety profile in older populations.
