This reduced-intensity conditioning (RIC) regimen is a combination therapy evaluated by the University Hospital Tuebingen for patients with acute myeloid leukemia (AML) or high-risk myelodysplastic syndrome (MDS) undergoing allogeneic hematopoietic stem cell transplantation (allo-HCT). The regimen consists of a standard backbone of fludarabine and treosulfan augmented with venetoclax, a selective B-cell lymphoma 2 (BCL-2) inhibitor. Venetoclax is added to enhance the anti-leukemic effect of the conditioning phase by inducing apoptosis in malignant cells, while fludarabine and treosulfan provide the necessary immunosuppression and myeloablation required for successful donor cell engraftment. This combination aims to reduce the risk of post-transplant relapse while maintaining a manageable safety profile compared to more intensive conditioning protocols.
