Venetoclax + navitoclax is a combination of two small molecule inhibitors targeting the B-cell lymphoma 2 (BCL-2) family of proteins, used primarily for the treatment of hematologic malignancies, including relapsed or refractory acute lymphoblastic leukemia (ALL) and lymphoblastic lymphoma (LL)[1][3][5][4]. Venetoclax is a selective inhibitor of BCL-2, promoting apoptosis in malignant cells dependent on BCL-2 for survival[1]. Navitoclax inhibits both BCL-2 and BCL-XL, broadening the apoptotic effect; however, navitoclax monotherapy is limited by dose-dependent thrombocytopenia[1][3][5]. Combining venetoclax with low-dose navitoclax aims to target both BCL-2 and BCL-XL with manageable toxicity profiles[3][5][4]. The combination is under investigation in phase I clinical trials, often administered with chemotherapy, and has demonstrated promising efficacy and tolerability in pediatric and adult patients, including those previously treated with transplants or immunotherapy[3][4][5]. The most common adverse effects include cytopenias, nausea, vomiting, febrile neutropenia, and delayed hematopoietic recovery[4].
