Overview

Verapamil + lithium is a combination of two pharmaceutical agents used primarily in the management of bipolar disorder, particularly for patients who do not respond adequately to lithium monotherapy. Verapamil is a calcium channel blocker that inhibits L-type calcium channels, leading to vasodilation and reduced cardiac workload; it is commonly used for hypertension, angina, and arrhythmias. Lithium is a mood stabilizer with complex mechanisms involving modulation of neurotransmitter release and inhibition of protein kinase C (PKC) signaling pathways. The combination has shown enhanced antimanic efficacy compared to either agent alone in some studies, possibly due to additive PKC inhibition. However, this combination increases the risk of adverse effects such as bradycardia, hypotension, neurotoxicity, and exacerbation of lithium side effects[1][2][5]. Careful monitoring is required when these drugs are co-administered.
