Overview

**VH4004280 + midazolam** refers to the co-administration of VH4004280 (also known as VH-280, a potent investigational HIV-1 capsid inhibitor developed for long-acting antiretroviral therapy) and midazolam (a well-established benzodiazepine used mainly as a probe for CYP3A4 activity and as a sedative/hypnotic). VH4004280 acts by inhibiting multiple steps in the HIV-1 life cycle, particularly after nuclear import and before integration, also impacting nuclear import, reverse transcript production, integration, virion assembly, and maturation. Clinical studies used midazolam alongside VH-280 to evaluate potential drug–drug interactions via CYP3A4; VH4004280 was shown not to significantly inhibit or induce CYP3A4 as determined by unchanged midazolam pharmacokinetics. VH4004280 is being developed by ViiV Healthcare, with development status at Phase 2 for HIV-1 infection. There is no evidence this specific combination is marketed as a fixed-dose product; rather, it is used in clinical pharmacokinetic interaction studies.
