Vigabatrin + prednisolone is a combination therapy used primarily for the treatment of infantile spasms (also known as West syndrome), a severe epileptic encephalopathy in infants. Vigabatrin is an irreversible inhibitor of gamma-aminobutyric acid transaminase (GABA-T), leading to increased levels of GABA, the main inhibitory neurotransmitter in the brain. Prednisolone is a synthetic glucocorticoid corticosteroid that exerts anti-inflammatory and immunosuppressive effects by modulating gene expression via the glucocorticoid receptor. The combination has been shown to be more effective than vigabatrin monotherapy for achieving cessation of spasms and improving electroencephalographic outcomes, with response rates up to 80% reported in some studies. This regimen may be particularly useful when adrenocorticotropic hormone (ACTH) is unavailable or not feasible[1][2][3][4][5][6].
