Vildagliptin + glimepiride is an oral combination of two antidiabetic agents used in the management of type 2 diabetes mellitus. **Vildagliptin** is a potent, selective inhibitor of dipeptidyl peptidase-4 (DPP-4), which increases the availability of endogenous incretin hormones, notably *glucagon-like peptide-1 (GLP-1)* and *glucose-dependent insulinotropic polypeptide (GIP)*, resulting in enhanced glucose-dependent insulin secretion and suppression of inappropriate glucagon release. **Glimepiride** is a sulfonylurea that stimulates pancreatic beta cells to release insulin independently of blood glucose concentration. Used together, their complementary mechanisms target both fasting and postprandial glucose levels, improving glycemic control in patients not adequately controlled with metformin plus sulfonylurea dual therapy.
