Vilobelimab + best supportive care is a combination regimen for severe COVID-19, specifically indicated for patients requiring invasive mechanical ventilation or extracorporeal membrane oxygenation. **Vilobelimab** is a chimeric monoclonal immunoglobulin G4 antibody that binds selectively with high affinity to soluble human complement component C5a, blocking its downstream inflammatory effects. This agent is administered in addition to best supportive care (SOC), which includes all standard therapies for critical COVID-19 as per clinical guidelines. The combination has demonstrated a statistically significant survival benefit and reduced need for renal replacement therapy in randomized controlled trials. Vilobelimab (marketed as Gohibic) was developed by InflaRx and has received emergency/conditional approval for the treatment of severe COVID-19 in select regions.
