This combination chemotherapy regimen is an intensive salvage treatment primarily utilized for patients with advanced germ cell tumors, such as testicular cancer, who have relapsed after or are refractory to standard first-line therapies like BEP (bleomycin, etoposide, and cisplatin). The regimen integrates four potent antineoplastic agents with distinct mechanisms of action to overcome drug resistance: vinblastine, a vinca alkaloid that disrupts microtubule assembly to arrest mitosis; etoposide, a topoisomerase II inhibitor that induces double-strand DNA breaks; ifosfamide, an alkylating agent that forms DNA cross-links; and cisplatin, a platinum-based agent that creates DNA adducts. Due to the high risk of ifosfamide-induced hemorrhagic cystitis, the regimen is typically administered alongside mesna for uroprotection, and intensive hydration is required to mitigate cisplatin-related nephrotoxicity. This protocol is a cornerstone of salvage oncology for germ cell malignancies, often achieving durable remissions in heavily pre-treated populations.
