Overview

This is a combination chemotherapy regimen consisting of three small-molecule cytotoxic agents—vincristine, cisplatin, and lomustine (also known as CCNU)—used primarily in the treatment of medulloblastoma and other central nervous system tumors. - **Vincristine** is a vinca alkaloid that inhibits microtubule formation in mitotic spindle, arresting cell division at metaphase. - **Cisplatin** is a platinum-based compound that forms DNA crosslinks and induces apoptosis by interfering with DNA replication and transcription. - **Lomustine (CCNU)** is an alkylating nitrosourea agent that causes DNA damage through alkylation and carbamoylation, leading to cell death. The combination exploits different mechanisms to maximize tumor cell kill while minimizing resistance. It has been widely used as adjuvant or maintenance therapy following surgery and radiotherapy for pediatric medulloblastoma, especially in high-risk or recurrent cases[1][2][3][6][7]. The regimen may be referred to as "CLV" (cisplatin-lomustine-vincristine) or "CCNU-cisplatin-vincristine" in clinical protocols.
