VEC + methotrexate is a multi-agent combination chemotherapy regimen primarily utilized in the treatment of pediatric ependymoma, specifically within the SIOP Ependymoma II clinical trial program. The regimen consists of a standard backbone of vincristine, etoposide, and cyclophosphamide (VEC), supplemented with high-dose methotrexate (HD-MTX) followed by folinic acid rescue. This combination employs multiple cytotoxic mechanisms to target tumor cells: vincristine inhibits microtubule polymerization, etoposide targets topoisomerase II to induce DNA damage, cyclophosphamide acts as a DNA-alkylating agent, and methotrexate inhibits dihydrofolate reductase (DHFR) to disrupt DNA synthesis. It is being evaluated for its efficacy in improving survival outcomes in children and young adults with residual inoperable disease following surgery.
