This refers to the combination of **vismodegib**, a small molecule inhibitor of the Hedgehog signaling pathway, and **rosiglitazone**, a thiazolidinedione class peroxisome proliferator-activated receptor gamma (PPARγ) agonist. Vismodegib selectively binds and inhibits the Smoothened protein, thereby blocking intracellular Hedgehog pathway signaling, which is implicated in the growth of basal cell carcinoma (BCC) and other cancers. Rosiglitazone acts as an agonist of PPARγ, influencing gene expression involved in glucose and lipid metabolism and is mainly indicated for type 2 diabetes mellitus. This combination has been studied in drug-drug interaction (DDI) trials to determine whether vismodegib alters the pharmacokinetics or efficacy of rosiglitazone, particularly via CYP2C8 inhibition. Notably, clinical pharmacokinetic studies found that vismodegib does not significantly alter exposure of rosiglitazone in patients with cancer, indicating a low potential for clinically relevant drug-drug interaction when the two drugs are co-administered[1][2][3].
