Overview

This is a **combination antithrombotic therapy** comprising a **vitamin K antagonist** (such as warfarin) and **aspirin**. Vitamin K antagonists inhibit the enzyme vitamin K epoxide reductase, reducing the synthesis of active, vitamin K-dependent coagulation factors II, VII, IX, and X, and proteins C and S, thereby producing anticoagulation[4][3]. Aspirin is an antiplatelet agent that irreversibly inhibits cyclooxygenase-1 (COX-1), reducing thromboxane A2 formation and platelet aggregation. The combination is used primarily in patients with conditions such as atrial fibrillation, mechanical heart valves, or those requiring oral anticoagulation who have also had a percutaneous coronary intervention (PCI) or acute coronary syndrome (ACS)[1][2]. The dual regimen increases bleeding risk compared to monotherapy or dual antiplatelet therapy, as both drugs impair different stages of hemostasis[1][2][4].
