Overview

This is a **four-drug combination** consisting of vonoprazan, esomeprazole, amoxicillin, and clarithromycin, used for the eradication of Helicobacter pylori (H. pylori) infection. - **Vonoprazan** is a potassium-competitive acid blocker (PCAB) that suppresses gastric acid secretion by competitively inhibiting the K+-binding site of the gastric H+,K+-ATPase enzyme. - **Esomeprazole** is a proton pump inhibitor (PPI) that irreversibly inhibits the same H+,K+-ATPase, reducing acid production in the stomach. - **Amoxicillin** is a beta-lactam antibiotic that inhibits bacterial cell wall synthesis. - **Clarithromycin** is a macrolide antibiotic that blocks bacterial protein synthesis by binding to the 50S ribosomal subunit. This combination therapy targets both acid suppression (maximizing the stability and efficacy of the antibiotics) and direct bacterial eradication, aiming to improve success rates in H. pylori elimination. Both **vonoprazan-based** and **esomeprazole-based** multi-drug regimens are used clinically, but there is minimal evidence for the four-drug combination of all these components together. The three-drug vonoprazan-based combination—without esomeprazole—has an FDA-approved branded version (Voquezna Triple Pak) for H. pylori[1][2][3][4][5][7]. However, published studies of quadruple therapy using both vonoprazan and esomeprazole with antibiotics are rare, and clinical practice typically uses one acid-suppressing agent with antibiotics, not both simultaneously[6][8].
