Overview

Vorinostat + etoposide is a combination of two small molecule chemotherapeutic agents used primarily in investigational regimens for hematologic malignancies such as acute leukemia. Vorinostat (suberoylanilide hydroxamic acid, SAHA) is a histone deacetylase (HDAC) inhibitor that targets class I and II HDAC enzymes. By inhibiting these enzymes, vorinostat leads to the accumulation of acetylated histones and other proteins, resulting in altered gene expression, cell cycle arrest, apoptosis, and inhibition of tumor growth[1][2][6]. Etoposide is a semisynthetic derivative of podophyllotoxin that inhibits DNA topoisomerase II. This action induces double-stranded DNA breaks and prevents repair by topoisomerase II binding, leading to cell death primarily during the S and G2 phases of the cell cycle[4]. The combination has been studied in phase I clinical trials for poor-risk or advanced acute leukemia to determine safety and preliminary efficacy[1][3].
