Vorinostat + gemcitabine + platinum-based agent is an investigational combination chemotherapy regimen consisting of vorinostat (a histone deacetylase inhibitor), gemcitabine (a nucleoside analog antimetabolite), and a platinum-based agent, commonly cisplatin or carboplatin. Vorinostat works by inhibiting histone deacetylases, leading to increased acetylation of histones and other proteins, which alters gene expression and can induce cancer cell death. Gemcitabine incorporates into DNA and inhibits DNA synthesis, leading to cell death. Platinum-based agents (such as cisplatin or carboplatin) form DNA crosslinks, which block replication and transcription, ultimately resulting in apoptotic cell death. This combination has been studied mainly in advanced non-small cell lung cancer and ovarian cancer settings, where preclinical and early clinical studies suggest vorinostat may increase the efficacy of gemcitabine and platinum-based chemotherapy by enhancing cytotoxicity and modulating DNA repair mechanisms[1][3].
