VSL#3 + mesalamine is a combination therapy consisting of a high-potency multi-strain probiotic (VSL#3) and the anti-inflammatory aminosalicylate drug mesalamine. VSL#3 contains eight live freeze-dried bacterial species (four strains of Lactobacillus, three strains of Bifidobacterium, and Streptococcus salivarius subsp. thermophilus), and modulates the intestinal barrier by enhancing tight junctions, supporting commensal bacterial populations, modulating mucin gene expression, and regulating immune pathways through NF-κB inhibition and anti-inflammatory cytokine induction. Mesalamine inhibits cyclooxygenase and lipoxygenase pathways to reduce prostaglandin and leukotriene synthesis, conferring anti-inflammatory actions in the gut. This combination strives to synergistically improve outcomes in inflammatory bowel disease, particularly ulcerative colitis, by targeting different pathogenic mechanisms: immunomodulation, barrier function support, and direct suppression of colonic inflammation[1][3][4].
