Overview

This is a combination therapy consisting of three agents: - **VSV-IFNβ-NIS** is a recombinant oncolytic vesicular stomatitis virus (VSV) engineered to express interferon-beta (IFNβ) and the sodium iodide symporter (NIS). The virus selectively infects and lyses cancer cells with defective interferon pathways, promoting oncolytic activity and enhancing antitumor immunity via IFNβ expression. The addition of NIS allows for imaging and potentially radioiodine therapy. - **Ipilimumab** is a fully human monoclonal antibody that targets Cytotoxic T-lymphocyte-associated protein 4 (CTLA-4), an inhibitory checkpoint on T cells, thereby enhancing the immune response against tumors. - **Nivolumab** is a fully human monoclonal antibody that targets Programmed cell death protein 1 (PD-1), another key immune checkpoint on T cells, further stimulating immune-mediated destruction of tumor cells. The rationale for this combination is that VSV-IFNβ-NIS promotes tumor cell lysis and local immune activation, while ipilimumab and nivolumab remove inhibitory signals from T cells, resulting in amplified and sustained antitumor immunity. This regimen is being investigated for its synergistic activity in solid tumors, particularly immunologically "cold" tumors such as melanoma, where preclinical and early clinical results have shown enhanced efficacy compared to monotherapy or dual checkpoint inhibition[1][4][5].
