Overview

Vudalimab + olaparib is a combination of two anticancer agents under clinical investigation for the treatment of metastatic castration-resistant prostate cancer (mCRPC), particularly in patients with homologous recombination deficiency or CDK12 biallelic loss. Vudalimab is a humanized bispecific monoclonal antibody that blocks both PD-1 and CTLA-4 checkpoint pathways, designed to promote T-cell mediated antitumor immunity by preferentially binding PD-1/CTLA-4 dual-positive cells. Olaparib is a small-molecule inhibitor of poly(ADP-ribose) polymerase (PARP), a key enzyme involved in DNA damage repair, with activity against tumors with homologous recombination repair defects. The combination aims to enhance immune-mediated tumor destruction and genomic instability in tumors, leveraging both checkpoint inhibition and synthetic lethality.
