Vyxeos + clofarabine is a combination chemotherapy regimen being investigated for the treatment of pediatric and young adult patients with relapsed or refractory acute myeloid leukemia (AML). Vyxeos (CPX-351) is a liposomal encapsulation of daunorubicin and cytarabine in a fixed 1:5 molar ratio, designed to optimize synergistic drug delivery while potentially reducing cardiotoxicity. Clofarabine is a second-generation purine nucleoside analogue that acts as a potent inhibitor of ribonucleotide reductase and DNA polymerase. The addition of clofarabine to the Vyxeos backbone is intended to enhance the accumulation of Ara-CTP (the active cytotoxic metabolite of cytarabine) within leukemic blasts, thereby increasing anti-tumor activity. This combination is currently being evaluated in the Phase 1b ITCC-092 trial sponsored by the Princess Maxima Center for Pediatric Oncology.
