Vyxeos + gilteritinib is a combination therapy being evaluated for the treatment of relapsed or refractory FLT3-mutated acute myeloid leukemia (AML). Vyxeos (CPX-351) is a liposomal encapsulation of daunorubicin and cytarabine in a fixed 1:5 molar ratio, designed to maintain synergistic drug concentrations in the bone marrow and tumor microenvironment. Daunorubicin is an anthracycline that inhibits topoisomerase II and intercalates DNA, while cytarabine is an antimetabolite that inhibits DNA polymerase. Gilteritinib is an oral small-molecule inhibitor of FMS-like tyrosine kinase 3 (FLT3) and AXL. This combination aims to provide cytotoxic chemotherapy alongside targeted inhibition of the FLT3 mutation, which is a common driver of poor prognosis and resistance in AML.
