warfarin + clopidogrel

This is a combination therapy consisting of warfarin, an oral anticoagulant (vitamin K antagonist), and clopidogrel, an oral antiplatelet agent. Warfarin inhibits the vitamin K epoxide reductase complex subunit 1 (VKORC1), thereby reducing the synthesis of active clotting factors II, VII, IX, and X as well as proteins C and S. This results in decreased blood coagulation. Clopidogrel is a prodrug that irreversibly inhibits the P2Y12 ADP receptor on platelets, preventing platelet aggregation. The combination is sometimes used in patients with indications for both anticoagulation (e.g., atrial fibrillation) and antiplatelet therapy (e.g., after vascular surgery or with coexisting coronary artery disease). However, this dual therapy significantly increases bleeding risk compared to monotherapy with either agent[2][3][7]. Clinical use requires careful patient selection and monitoring.
