warfarin + low molecular weight heparin

A combination anticoagulant therapy that pairs warfarin (a vitamin K antagonist) with low molecular weight heparin (LMWH). Warfarin inhibits vitamin K-dependent clotting factors, while LMWH activates antithrombin to inactivate coagulation enzymes, particularly factor Xa. This combination is primarily used during initiation of oral anticoagulation therapy until warfarin reaches therapeutic levels (INR 2-3) or as bridging therapy during temporary warfarin interruptions. Co-administration enhances profibrinolytic effects through both TAFI-dependent and TAFI-independent mechanisms.
