This is a combination therapy of warfarin, an oral vitamin K antagonist anticoagulant, and low molecular weight heparin (LMWH), a parenteral anticoagulant. Warfarin inhibits the synthesis of vitamin K-dependent clotting factors (II, VII, IX, X) in the liver by blocking vitamin K epoxide reductase. LMWHs (such as enoxaparin or dalteparin) are derived from standard heparin and primarily inhibit factor Xa via antithrombin III activation; they have more predictable pharmacokinetics than unfractionated heparin. The combination is commonly used as bridging therapy during initiation or temporary interruption of warfarin treatment to ensure continuous anticoagulation due to the delayed onset of action of warfarin. Co-administration reduces fibrinolytic resistance through both TAFI-dependent and independent mechanisms[1][4][7]. This regimen is especially relevant for patients with acute venous thromboembolism (VTE), including those with cancer[3][5].
