White blood cells + granulocytes is an experimental allogeneic cellular immunotherapy developed by Dr. Dipnarine Maharaj at the South Florida Bone Marrow/Stem Cell Transplant Institute. The therapy consists of HLA-mismatched, non-irradiated white blood cells, predominantly granulocytes (68–91%), collected via leukapheresis from young, healthy donors following mobilization with granulocyte colony-stimulating factor (G-CSF) and dexamethasone. The treatment is based on the hypothesis that granulocytes from certain individuals possess innate cancer-killing activity (CKA), a concept derived from the study of spontaneous regression/complete resistance (SR/CR) mice at Wake Forest University. By using HLA-mismatched donors, the therapy aims to avoid transfusion-related graft-versus-host disease (GvHD) while leveraging the innate immune system to recognize and destroy solid tumor cells. It is currently being investigated in a Phase I/II clinical trial for various metastatic or unresectable solid tumors, including pancreatic, breast, and lung cancers.
