WT1 peptide (modified) is an HLA-A*2402-restricted cancer vaccine consisting of a modified 9-mer peptide (CYTWNQMNL) derived from the Wilms' tumor 1 (WT1) protein. The peptide features a single amino acid substitution at the second position, where the natural cysteine is replaced by tyrosine (C→Y). This modification significantly enhances the binding affinity of the peptide to the HLA-A*2402 MHC class I molecule, thereby increasing its immunogenicity compared to the natural sequence. When administered, typically as an emulsion with the adjuvant Montanide ISA51 via intradermal injection, the vaccine stimulates the activation and expansion of WT1-specific cytotoxic T lymphocytes (CTLs). These CTLs are capable of recognizing and eliminating tumor cells that overexpress the WT1 protein, which is a common feature in various hematological malignancies and solid tumors. The vaccine has been primarily developed and clinically evaluated in Japan for indications such as leukemia, myelodysplastic syndromes, and glioblastoma.
