XELOX + QL1706 + tyrosine kinase inhibitor is a combination therapy regimen being evaluated for the treatment of advanced hepatocellular carcinoma (HCC). The regimen consists of the XELOX chemotherapy backbone (oxaliplatin and capecitabine), the bifunctional anti-PD-1/CTLA-4 antibody QL1706 (also known as iparomlimab/tuononlimab), and a tyrosine kinase inhibitor (TKI), specifically either lenvatinib or regorafenib. QL1706 is a MabPair bifunctional antibody that simultaneously blocks the Programmed Cell Death Protein 1 (PD-1) and Cytotoxic T-Lymphocyte Associated Protein 4 (CTLA-4) immune checkpoints, aiming to enhance the anti-tumor immune response. The TKI component (lenvatinib or regorafenib) provides anti-angiogenic and anti-proliferative effects by inhibiting multiple receptor tyrosine kinases, including VEGFR and FGFR. The XELOX component provides cytotoxic chemotherapy. This combination is part of the HCC-SIGHT platform trial led by Fudan University to identify effective second-line therapies for HCC.
