Xeruborbactam + cefiderocol is an investigational combination therapy targeting multidrug-resistant Gram-negative bacterial infections. Cefiderocol is a siderophore cephalosporin antibiotic that utilizes iron transport channels to enter bacterial cells and inhibits penicillin-binding proteins, thereby blocking cell wall synthesis and causing bacterial death. Xeruborbactam is a novel, ultra-broad-spectrum boron-based beta-lactamase inhibitor that inactivates both serine-type and metallo-beta-lactamases, enzymes responsible for resistance to many beta-lactams. The combination aims to restore or enhance the activity of cefiderocol against bacteria producing these resistance enzymes, including carbapenem-resistant strains such as *Acinetobacter baumannii* and *Enterobacterales*. This combination is being developed by Qpex Biopharma (a Shionogi Group company) with support from BARDA for the treatment of serious drug-resistant Gram-negative infections[1][2][4].
