Xylocarn (xylitol + levocarnitine) is an intravenous combination therapy developed by Iperboreal Pharma for the management of complications in patients with end-stage kidney disease (ESKD) undergoing maintenance hemodialysis. The formulation combines xylitol, a five-carbon sugar alcohol, with levocarnitine (L-carnitine), an amino acid derivative essential for fatty acid metabolism. Xylitol serves as an osmotic agent to stabilize plasma osmolality and prevent intradialytic hypotension (IDH), while also providing a non-insulin-dependent energy source. Levocarnitine addresses the systemic carnitine deficiency common in dialysis patients, supporting myocardial function and energy production by facilitating the transport of long-chain fatty acids into the mitochondria. The combination is designed to be administered during the dialysis session to improve metabolic stability and reduce the incidence of blood pressure drops.
