TLX66 (yttrium (90Y) besilesomab) is a radioimmunotherapy agent comprising the murine IgG1 monoclonal antibody besilesomab radiolabelled with the beta-emitting isotope yttrium-90. The antibody component, besilesomab, targets CD66 (specifically the CD66b/NCA-95 isoform), a cell surface antigen highly expressed on granulocytes and their bone marrow progenitor cells. By binding to these targets, the drug enables the selective delivery of high-energy beta radiation directly to the bone marrow compartment. It is primarily being developed as a targeted conditioning regimen to facilitate bone marrow ablation prior to autologous stem cell transplantation (ASCT) in patients with systemic AL amyloidosis and multiple myeloma. This approach aims to provide a more precise and potentially less toxic alternative to conventional high-dose chemotherapy conditioning regimens like melphalan.
