Yttrium-90 microsphere therapy, also known as Selective Internal Radiation Therapy (SIRT) or Transarterial Radioembolization (TARE), is a localized radiopharmaceutical treatment for unresectable liver tumors. It involves the delivery of millions of radioactive microspheres (either glass or resin) containing the beta-emitting isotope Yttrium-90 directly into the hepatic artery. These microspheres lodge in the tumor's microvasculature, delivering high-dose, short-range radiation that induces DNA damage and cell death in malignant tissues while minimizing exposure to healthy liver parenchyma. The therapy is primarily used for hepatocellular carcinoma (HCC) and metastatic liver disease, such as colorectal cancer metastases. It is classified as a brachytherapy device and radiopharmaceutical.
