Overview
Zabinostat + pembrolizumab is a combination therapy comprising the small-molecule histone deacetylase (HDAC) inhibitor zabinostat (also known as CXD101 or zabadinostat) and the anti-PD-1 monoclonal antibody pembrolizumab. Zabinostat acts as an epigenetic immune regulator, enhancing the expression of genes involved in tumor antigen presentation and potentially sensitizing tumor cells to immune-mediated destruction. Pembrolizumab blocks the interaction between PD-1 on T-cells and its ligands (PD-L1/PD-L2) on tumor cells, thereby reversing immune evasion and boosting anti-tumor immune responses. The combination is primarily being evaluated in clinical trials, such as the Phase Ib/II PLACARD study, for the treatment of patients with relapsed or refractory diffuse large B-cell lymphoma (DLBCL) and microsatellite stable (MSS) colorectal cancer, with the rationale that zabinostat's epigenetic modulation can overcome resistance to checkpoint blockade.
