Zanidatamab + chemotherapy is a combination therapy consisting of zanidatamab, a humanized bispecific biparatopic monoclonal antibody, and various standard-of-care cytotoxic chemotherapy agents. Zanidatamab, developed by Zymeworks and licensed to Jazz Pharmaceuticals, utilizes the Azymetric™ platform to simultaneously bind two non-overlapping epitopes of the Human Epidermal Growth Factor Receptor 2 (HER2), specifically the juxtamembrane domain (ECD4) and the dimerization domain (ECD2). This dual binding results in multiple mechanisms of action, including potent HER2 signal blockade, enhanced receptor clustering and internalization from the cell surface, and strong effector functions such as antibody-dependent cellular cytotoxicity (ADCC). When combined with chemotherapy, the regimen aims to provide synergistic antitumor activity. It is currently being investigated in Phase 1 and Phase 3 trials for patients with advanced HER2-expressing cancers, including those in the third-line (3L) setting who have progressed on prior therapies.
