Overview

Zenocutuzumab + endocrine therapy is a combination regimen used primarily in the investigational and early clinical setting for the treatment of **estrogen receptor-positive (ER+), low HER2 expression metastatic breast cancer** and potentially other solid tumors with **NRG1 gene fusion**. - **Zenocutuzumab** is a bispecific monoclonal antibody that targets and inhibits both HER2 and HER3 receptors, blocking oncogenic signaling from NRG1 fusion proteins, and also induces antibody-dependent cellular cytotoxicity. - **Endocrine therapy** generally refers to agents (e.g., aromatase inhibitors, selective estrogen receptor modulators/degraders) that antagonize or downregulate estrogen receptor signaling, a key driver in ER+ breast cancer. The combination aims to simultaneously inhibit both the hormone (estrogen receptor) and growth factor (HER2/HER3) pathways, which are often co-activated in advanced ER+/low HER2 breast cancers, and to address resistance to single-agent therapy. This regimen is currently under clinical investigation and is not approved as a combination; however, zenocutuzumab as monotherapy has received accelerated approval for NRG1 fusion-positive NSCLC and pancreatic cancer.
