Zilovertamab + docetaxel is an investigational combination therapy being evaluated for the treatment of metastatic castration-resistant prostate cancer (mCRPC). Zilovertamab (formerly cirmtuzumab) is a humanized monoclonal antibody that specifically targets Receptor Tyrosine Kinase Like Orphan Receptor 1 (ROR1), an onco-embryonic surface protein that is highly expressed in various malignancies, including prostate cancer, but is not expressed on normal adult tissues. By binding to ROR1, zilovertamab inhibits the non-canonical Wnt5a signaling pathway, which drives tumor cell survival, stemness, and metastasis. Docetaxel is a standard-of-care taxane chemotherapy that binds to beta-tubulin, stabilizing microtubules and preventing their disassembly, which leads to cell cycle arrest in the G2/M phase and subsequent apoptosis. The combination is hypothesized to enhance anti-tumor activity by simultaneously inhibiting ROR1-mediated survival signals and inducing mitotic arrest.
