ziv-aflibercept + capecitabine + oxaliplatin

ziv-aflibercept + capecitabine + oxaliplatin is a combination therapy regimen consisting of a biologic anti-angiogenic agent and two chemotherapy drugs, evaluated as a first-line treatment for metastatic colorectal cancer (mCRC). Ziv-aflibercept (Zaltrap) is a recombinant fusion protein that acts as a VEGF trap, binding to vascular endothelial growth factor (VEGF)-A, VEGF-B, and placental growth factor (PlGF) with high affinity to inhibit tumor angiogenesis. This is combined with the XELOX (or CapeOx) chemotherapy backbone, which includes capecitabine—an oral prodrug of 5-fluorouracil that inhibits thymidylate synthase to disrupt DNA synthesis—and oxaliplatin, a platinum-based agent that forms DNA crosslinks to induce apoptosis. In the Phase II clinical trial (NCT01744249) sponsored by James J Lee, Arm B specifically refers to a dosing schedule where all three agents are administered every three weeks (q3w), with ziv-aflibercept dosed at 6 mg/kg.
