zolbetuximab + zoledronic acid

Zolbetuximab + zoledronic acid is a combination therapy investigated for the treatment of advanced gastric and gastroesophageal junction (G/GEJ) adenocarcinoma. The regimen consists of zolbetuximab (formerly IMAB362), a chimeric IgG1 monoclonal antibody targeting Claudin 18.2 (CLDN18.2), and zoledronic acid, a bisphosphonate typically used for bone-related conditions. This combination was evaluated in the Phase I PILOT trial (NCT01671774) alongside interleukin-2 (IL-2). The scientific rationale was that zoledronic acid and IL-2 could enhance zolbetuximab's antibody-dependent cellular cytotoxicity (ADCC) by activating and expanding γδ T cells and natural killer cells. While zolbetuximab has since been approved as a monotherapy and in other combinations (e.g., with chemotherapy), this specific combination with zoledronic acid did not demonstrate significant additional clinical benefit in early-stage trials and was not pursued further for these indications.
