This combination is an investigational regimen comprising **zongertinib** (a selective, irreversible HER2 tyrosine kinase inhibitor), **trastuzumab** (a monoclonal antibody targeting the HER2 extracellular domain), and **capecitabine** (an oral prodrug of 5-fluorouracil that inhibits DNA synthesis). Zongertinib blocks mutated HER2 receptor signaling, which drives uncontrolled cell proliferation in HER2-mutated cancers. Trastuzumab binds the HER2 receptor, blocking downstream signaling and inducing antibody-dependent cellular cytotoxicity. Capecitabine is metabolized to 5-FU, incorporating into DNA/RNA and inhibiting thymidylate synthase, resulting in cytotoxicity. Investigated for unresectable or metastatic non-squamous non-small cell lung cancer and HER2+ metastatic cancers (including breast, gastric, gastroesophageal junction, and esophageal adenocarcinomas) where prior therapies have failed. Zongertinib is FDA-approved for NSCLC with HER2 TKD mutations (monotherapy), while this triple-drug regimen is in phase Ib/II clinical development for HER2+ metastatic solid tumors.
