Overview

The 1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid (DOTA)-hapten chelate is a synthetic molecular complex used as a specialized ligand in pre-targeted radioimmunotherapy (PRIT) (Cheal et al., 2016). It consists of the DOTA macrocycle, which coordinates a therapeutic or diagnostic radioisotope (e.g., Lutetium-177, Yttrium-90, or Actinium-225), and a hapten moiety recognized with picomolar affinity by an engineered antibody (Schlom & Carrasquillo, 2020). In clinical applications, a bispecific antibody is first administered to bind a tumor-associated antigen, such as GPA33 or GD2; after the antibody has cleared from the blood but remains on the tumor, the DOTA-hapten is injected (Villar et al., 2022). The DOTA-hapten is then captured by the antibody at the tumor site, while unbound molecules are rapidly excreted through the kidneys. This approach significantly improves the therapeutic index by decoupling the slow pharmacokinetics of antibodies from the radioactive decay of the payload, thereby reducing off-target radiation to the bone marrow (Cheal et al., 2017). The DOTA-hapten chelate is a central component of modular theranostic platforms currently being evaluated for the treatment of various solid tumors and pediatric neuroblastoma (MSKCC, 2023).
