The 5T2MM M-protein is a monoclonal immunoglobulin produced by malignant plasma cells in the 5T2MM murine model, which serves as a syngeneic and immunocompetent representation of human multiple myeloma [1, 4]. This protein features a unique idiotype—the specific antigen-binding region of the antibody—that acts as a highly specific tumor-associated antigen [4, 14]. In preclinical research, the 5T2MM M-protein is utilized as a target for anti-idiotypic therapeutic strategies, including targeted radionuclide therapy (TRNT) and the development of CAR-T cells [12, 14]. Single-domain antibodies (nanobodies), such as R3B23, have been engineered to bind specifically to this idiotype, allowing for the delivery of cytotoxic radioisotopes directly to the tumor cells [4, 12]. While the high specificity of the idiotype reduces off-target toxicity, the secretion of the M-protein into the bloodstream can create a sink effect, where circulating proteins sequester the drug before it reaches the bone marrow [4, 14]. Additionally, monitoring serum levels of the 5T2MM M-protein is a critical biomarker for assessing tumor burden and the efficacy of experimental treatments in this model [1, 3].
