Acetate metabolism as a target for acid-base balance refers to the use of acetate as a bicarbonate precursor in balanced intravenous fluids. When administered, acetate is rapidly metabolized—primarily in muscle and other tissues—into bicarbonate, which helps correct metabolic acidosis and maintain physiological pH. Unlike lactate, acetate metabolism is not dependent on liver function, making it a preferable choice in patients with hepatic impairment. Acetate enters cells, is converted to acetyl-CoA, enters the Krebs cycle, and ultimately generates bicarbonate ions, exerting an alkalinizing effect on body fluids.
