Overview

Germ cell alkaline phosphatase (GCAP), also known as the Nagao isoenzyme, is a membrane-bound enzyme encoded by the ALPG gene and is a member of the alkaline phosphatase family (UniProt P10696). It is primarily expressed in the cell membranes of primordial germ cells, the fetal testis, and the thymus, and it shares approximately 98% sequence identity with placental alkaline phosphatase (PLAP) (Millán, 2006). GCAP is a significant clinical biomarker, as it is highly overexpressed in certain malignancies, particularly seminomas, dysgerminomas, and embryonal carcinomas (NCBI Gene ID 251). Its biological role involves the hydrolysis of phosphate esters at alkaline pH, though its specific endogenous substrates in vivo are not fully characterized. In the context of drug development, GCAP has been explored as a target for monoclonal antibody-based imaging and therapy, such as radioimmunotherapy, due to its restricted expression in normal adult tissues. However, therapeutic applications are complicated by its high homology with other alkaline phosphatase isoforms and the fact that serum levels can be elevated in heavy smokers, reducing its specificity as a diagnostic marker (PubMed: 2835444).
