Overview

Allergen-specific Immunoglobulin E (IgE) antibodies against cow's milk proteins are the primary mediators of Type I hypersensitivity reactions in cow's milk allergy (CMA). These antibodies are produced by B cells following sensitization to specific milk proteins, primarily caseins (Bos d 8) and whey proteins such as alpha-lactalbumin (Bos d 4) and beta-lactoglobulin (Bos d 5). Once produced, these specific IgE molecules circulate and bind to high-affinity FcεRI receptors on the surface of mast cells and basophils. Upon subsequent exposure to cow's milk allergens, the proteins cross-link the surface-bound IgE, triggering the immediate release of inflammatory mediators like histamine and leukotrienes. This process leads to clinical symptoms ranging from mild urticaria to life-threatening anaphylaxis. Therapeutic interventions, such as the monoclonal antibody omalizumab, target these antibodies by neutralizing free IgE and downregulating its receptors, thereby increasing the threshold for allergic reactions and facilitating safer oral immunotherapy.
