Overview

The iron-responsive element (IRE) in the 5'-untranslated region (5'-UTR) of alpha-synuclein (SNCA) mRNA is a conserved RNA stem-loop structure that regulates the translation of alpha-synuclein in response to cellular iron levels (Rogers et al., 2011, J. Neurochem.). Under low-iron conditions, iron-regulatory proteins (IRPs) bind to this IRE, physically blocking the recruitment of the 43S pre-initiation complex and inhibiting translation (Moussaud et al., 2014, Exp. Neurol.). Conversely, high iron levels cause IRPs to dissociate, leading to increased alpha-synuclein synthesis, which can contribute to protein aggregation in neurodegenerative diseases (Favetta et al., 2019, Front. Neurosci.). This target is of significant interest in the treatment of synucleinopathies, such as Parkinson's disease and Lewy body dementia, where alpha-synuclein accumulation is a hallmark pathology (Chen et al., 2021, J. Biol. Chem.). Small molecule drugs like Posiphen (Buntanetap) are designed to bind to or stabilize the IRE-IRP complex, thereby reducing the translation of SNCA mRNA and lowering total alpha-synuclein protein levels (Annovis Bio, 2023). By decreasing the concentration of alpha-synuclein, these therapies aim to prevent the formation of toxic oligomers and insoluble fibrils that lead to neuronal death (Moussaud et al., 2014, Exp. Neurol.). This approach represents a disease-modifying strategy that targets the root cause of protein misfolding by controlling protein production at the translational level (Rogers et al., 2011, J. Neurochem.).
