Overview

Alveolar epithelial type II (ATII) cells are specialized cuboidal cells that play a critical role in maintaining pulmonary homeostasis. Their primary function is the production and secretion of pulmonary surfactant, which reduces surface tension at the air-liquid interface to prevent alveolar collapse (StatPearls, 2023). Beyond surfactant metabolism, ATII cells act as the stem cells of the alveolar epithelium, proliferating and differentiating into type I cells to repair damage (NIH, 2021). In clinical contexts, ATII cell dysfunction is central to the pathogenesis of Idiopathic Pulmonary Fibrosis (IPF), where chronic injury and senescence drive fibrotic responses, and they are the primary targets for SARS-CoV-2 infection (Nature, 2020). Therapeutic strategies involving ATII cells include exogenous surfactant administration for neonatal distress and the use of antifibrotic drugs like Nintedanib to mitigate epithelial-mesenchymal transition (PubMed, 2019).

Mechanism of action

Surfactant replacement therapy restores alveolar surface tension; tyrosine kinase inhibitors like Nintedanib target growth factor signaling pathways within these cells to reduce fibrotic progression (PubMed, 2019); viral entry inhibitors block receptors such as ACE2 located on the ATII cell surface (Nature, 2020).
